Track V:  Systems Change (C)

C1.  State Policy Update:

Important Legislation, Case Law and Planning that will Affect the Future of Supportive Housing in the State:  Presenters will highlight the most pertinent State legislation, case law and planning efforts that affect the permanent supportive housing field including a discussion of COAH and the new Third Round rules.

C2.  Breaking the Cycle of Frequent Users of Crisis Services: 
Many people experiencing homelessness cycle between crisis services like emergency departments, hospitals, jails and shelters.  Frequent users have complex issues that make it hard to break the cycle.  This workshop will present some successful programs that provide coordinated care and services to reduce the use of crisis services among the homeless through collaboration on a local level to ultimately break the cycle.

C3.  Local Systems Change: Moving from Managing Homelessness to Ending it:  More and more focus has been given to 10 year plans to end homelessness and moving from just managing homelessness to ending it. This workshop will focus on the concrete steps a community can take to actually achieve that goal.  A panel of community providers and leaders will present how they have made local systems change to implement the necessary steps to end homelessness in their community.

C4.  Moving on from Permanent Supportive Housing: 
Although tenant may remain in permanent supportive housing permanently, as tenants increase their independence they may be best served in other types of housing.  Presenters will describe how to assess tenants to see if they are appropriate to move on, how to encourage people to move on and what services are needed to ensure the success of the person leaving permanent supportive housing.
